Park Avenue Senior Care offers nursing care, case management, social and recreational activities and nutritional meals. Our nurses and nursing assistants are special training in:

• Medication oversight and Administration

• Vital sign monitoring

• Dressing changes

• incontinent care, Diabetes management

• Nutritional monitoring

Arts and Crafts; Dancing and Singing groups, Health Education and promotion; Cultural holiday celebration program.
 

Recreation of Activities:

Park Avenue has carefully selected the caterer to ensure that all meals are nutritionally satisfying. Our nurse and staff monitor each meal paying special attention to individual diet and requests. We offer light breakfast, lunch and snacks during the day which is included as part of the program.
 

We partner with Hope Care Ride to offer free transportation to all participants, from home to center and back home, to doctor’s visits and treatments. Our staff work with participants and family to ensure smooth transportation.
